Registered Nurse - care coordination (remote)

Fernwood Wilder Learning Group is an education provider in Toledo serving students and families. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 30 people, working mostly from home. What you will do • Support work on enrolment with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with enrolment. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with a monthly team day in Toledo. • We reply to every applicant.
