Registered Nurse - care coordination (remote)

Ironwood Birch Care Services provides in-home and community support in Wichita, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Support work on care plans with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Wichita. • We will tell you what to expect at each step of the hiring process.
