Registered Nurse - care coordination (remote)

Bluepeak Harbor Transit Planning Group is looking for a Registered Nurse. The job centres on care coordination for route schedules, and it would suit someone who likes steady, practical work. You would join a large organisation of about 626 people, working mostly from home. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on route schedules with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with a monthly team day in Chicago. • We reply to every applicant.
