Registered Nurse - care coordination (remote)

Stonegate Bridge Transit Planning Group is a transit-planning firm in Wichita working with agencies and riders, and we need a Registered Nurse to lead our care coordination. Work is collaborative and map-heavy. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on ridership counts with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Wichita. • Applications are reviewed on a rolling basis.
