Registered Nurse - care coordination (remote)

We are growing our team at Nova Sparrow Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 45 people, working mostly from home. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on fare systems with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with fare systems. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with a monthly team day in Des Moines. • We will tell you what to expect at each step of the hiring process.
