Registered Nurse - care coordination (remote)

Rivermark Timber Learning Group runs classrooms and tutoring across the Cleveland area. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on learning materials with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with learning materials. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Cleveland. • We will tell you what to expect at each step of the hiring process.
