Registered Nurse - care coordination (remote)

Fernwood Corner Learning Group is an education provider in Oakland serving students and families, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on student accounts with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with student accounts. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with an occasional day on site in Oakland. • We will tell you what to expect at each step of the hiring process.
