Registered Nurse - care coordination (remote)

Rivermark Fern Hospitality is a hospitality company in Nashville with restaurants, rooms and catering, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of about 654 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on reviews and feedback with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with reviews and feedback. Working arrangement • Part-time, about 16 hours a week, afternoons. • Remote-friendly, with a monthly team day in Nashville. • We reply to every applicant.
