Registered Nurse - care coordination (remote)

Highbridge Keystone Transit Planning plans bus and rail service for cities around Nashville, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 25 people, working mostly from home. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on route schedules with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with route schedules. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Nashville. • We will tell you what to expect at each step of the hiring process.
