Registered Nurse - care coordination (remote)

Summit Mill Care Services Group supports families and individuals in Chicago with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on family communication with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Chicago. • We will tell you what to expect at each step of the hiring process.
