Registered Nurse - care coordination (remote)

Bluepeak Linden Health is looking for a Registered Nurse. The job centres on care coordination for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Support work on patient scheduling with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with patient scheduling. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Des Moines. • Applications are reviewed on a rolling basis.
