Registered Nurse - care coordination (remote)

We are growing our team at Harborlight Sparrow Care Services and adding a Registered Nurse whose main responsibility will be care coordination. Teams support each other, especially on hard days. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on client records with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with client records. • BLS/ACLS certification. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Boston. • We reply to every applicant.
