Registered Nurse - care coordination (remote)

Northwind Forge Transit Planning is looking for a Registered Nurse. The job centres on care coordination for route schedules, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on route schedules with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with route schedules. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Des Moines. • Applications are reviewed on a rolling basis.
