Registered Nurse - care coordination (remote)

Clearpath Glen Health Group provides primary and specialty care in Fresno. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 192 people, working mostly from home. What you will do • Support work on referrals with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with referrals. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Fresno. • Applications are reviewed on a rolling basis.
