Registered Nurse - care coordination (remote)

We are growing our team at Ironwood Basin Care Services Group and adding a Registered Nurse whose main responsibility will be care coordination. We train and promote from within. What you will do • Support work on visit scheduling with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with visit scheduling. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Charleston. • We will tell you what to expect at each step of the hiring process.
