Registered Nurse - care coordination (remote)

Foxglove Summit Learning Group operates schools and learning programmes around San Diego, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 120 people, working mostly from home. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on student accounts with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with student accounts. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in San Diego. • We reply to every applicant.
