Registered Nurse - care coordination (remote)

Bright Cedar Health provides primary and specialty care in Minneapolis, and we need a Registered Nurse to lead our care coordination. Continuing education is supported. What you will do • Support work on referrals with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with referrals. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
