Registered Nurse - care coordination (remote)

Northwind Forge Transit Planning is a transit-planning firm in San Diego working with agencies and riders. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on route schedules with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with an occasional day on site in San Diego. • We will tell you what to expect at each step of the hiring process.
