Registered Nurse - care coordination (remote)

Fernwood Lakeside Transit Planning Group plans bus and rail service for cities around Charleston. We are hiring a Registered Nurse to take on care coordination. We value plain explanations. What you will do • Support work on route schedules with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Charleston. • Applications are reviewed on a rolling basis.
