Registered Nurse - care coordination (remote)

Northwind Sparrow Care Services Group supports families and individuals in Minneapolis with personal care and coordination. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of about 931 people, working mostly from home. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on licensing with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Minneapolis. • We will tell you what to expect at each step of the hiring process.
