Registered Nurse - care coordination (remote)

We are growing our team at Rivermark Stone Transit Planning Partners and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on survey data with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Jacksonville. • We reply to every applicant.
