Registered Nurse - care coordination (remote)

We are growing our team at Clearpath Drift Health Group and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of about 258 people, working mostly from home. What you will do • Follow infection-control and documentation standards. • Support work on referrals with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with referrals. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in El Paso. • Applications are reviewed on a rolling basis.
