Registered Nurse - care coordination (remote)

Ironwood Harbor Health Group provides primary and specialty care in Colorado Springs, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 193 people, working mostly from home. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on electronic records with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with electronic records. • BLS/ACLS certification. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
