Registered Nurse - care coordination (remote)

Highbridge Birch Care Services Group provides in-home and community support in Rochester. We are hiring a Registered Nurse to take on care coordination. You would join a small organisation of about 35 people, working mostly from home. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on caregiver training with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with caregiver training. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Rochester. • We will tell you what to expect at each step of the hiring process.
