Registered Nurse - care coordination (remote)

Amberfield Mill Care Services is a social-care provider serving older adults and people with disabilities around Jacksonville. We are hiring a Registered Nurse to take on care coordination. Teams support each other, especially on hard days. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on licensing with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Jacksonville. • Applications are reviewed on a rolling basis.
