Registered Nurse - care coordination (remote)

Amberfield Orchard Care Services is a social-care provider serving older adults and people with disabilities around Jacksonville. We are hiring a Registered Nurse to take on care coordination. We train and promote from within. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with a monthly team day in Jacksonville. • We will tell you what to expect at each step of the hiring process.
