Registered Nurse - care coordination (remote)

Westbrook Harbor Transit Planning is looking for a Registered Nurse. The job centres on care coordination for survey data, and it would suit someone who likes steady, practical work. You would join a large organisation of about 806 people, working mostly from home. What you will do • Support work on survey data with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with survey data. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Milwaukee. • We will tell you what to expect at each step of the hiring process.
