Registered Nurse - care coordination (remote)

Foxglove Ivory Health Partners runs clinics and patient services around Hartford. We are hiring a Registered Nurse to take on care coordination. Clinicians and administrators work side by side. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on referrals with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with referrals. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with a monthly team day in Hartford. • We reply to every applicant.
