Registered Nurse - care coordination (Raleigh, NC)

Fernwood Thistle Learning Group Associates operates schools and learning programmes around Raleigh. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of more than 1,000 people based in Raleigh. What you will do • Assess patients, record findings and carry out care plans. • Support work on school calendars with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with school calendars. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Raleigh. • We reply to every applicant.
