Registered Nurse - care coordination (Portland, OR)

Fernwood Juniper Transit Planning Associates is looking for a Registered Nurse. The job centres on care coordination for accessibility audits, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Support work on accessibility audits with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with accessibility audits. • BLS/ACLS certification. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Portland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
