Registered Nurse - care coordination (Portland, OR)

We are growing our team at Stonegate Wilder Insurance Group and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on premium billing with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with premium billing. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Portland. • Applications are reviewed on a rolling basis.
