Registered Nurse - care coordination (Pittsburgh, PA)

Harborlight Summit Learning Group is an education provider in Pittsburgh serving students and families, and we need a Registered Nurse to lead our care coordination. Collaboration between teachers and staff is strong. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • On site in Pittsburgh. • We will tell you what to expect at each step of the hiring process.
