Registered Nurse - care coordination (Pittsburgh, PA)

We are growing our team at Westbrook North Learning Group and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 17 people based in Pittsburgh. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with family communication. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
