Registered Nurse - care coordination (Pittsburgh, PA)

Fernwood Harbor Care Services is looking for a Registered Nurse. The job centres on care coordination for client records, and it would suit someone who likes steady, practical work. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on client records with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with client records. • BLS/ACLS certification.
