Registered Nurse - care coordination (Pittsburgh, PA)

We are growing our team at Redstone Willow Learning Group Partners and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Support work on family communication with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Full-time, on a regular day shift. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
