Registered Nurse - care coordination (Pittsburgh, PA)

Amberfield Cobalt Early Learning runs childcare centres in Pittsburgh caring for infants, toddlers and preschoolers. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of about 285 people based in Pittsburgh. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication.
