Registered Nurse - care coordination (Phoenix, AZ)

Cedar Hollow Transit Planning is looking for a Registered Nurse. The job centres on care coordination for service alerts, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on service alerts with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with service alerts. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Phoenix, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
