Registered Nurse - care coordination (Phoenix, AZ)

Northwind Basin Care Services is looking for a Registered Nurse. The job centres on care coordination for caregiver training, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on caregiver training with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with caregiver training.
