Registered Nurse - care coordination (Phoenix, AZ)

Rivermark Fox Learning Group operates schools and learning programmes around Phoenix. We are hiring a Registered Nurse to take on care coordination. You would join a large organisation of more than 1,000 people based in Phoenix. What you will do • Educate patients and families in plain language. • Support work on learning materials with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with learning materials. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Phoenix, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
