Registered Nurse - care coordination (Phoenix, AZ)

Amberfield Orchard Care Services provides in-home and community support in Phoenix, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of more than 1,000 people based in Phoenix. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on caregiver training with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with caregiver training. • BLS/ACLS certification. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Phoenix. • We will tell you what to expect at each step of the hiring process.
