Registered Nurse - care coordination (Phoenix, AZ)

Rivermark Cedar Health is looking for a Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. You would join a small organisation of about 43 people based in Phoenix. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on patient education with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with patient education. • BLS/ACLS certification.
