Registered Nurse - care coordination (Oakland, CA)

We are growing our team at Fernwood Fox Learning Group and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Oakland, with some flexibility after the first few months. • We reply to every applicant.
