Registered Nurse - care coordination (Oakland, CA)

Stonegate Mill Care Services Group is a social-care provider serving older adults and people with disabilities around Oakland, and we need a Registered Nurse to lead our care coordination. The work is personal and so is the culture. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on visit scheduling with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Oakland. • We reply to every applicant.
