Registered Nurse - care coordination (Oakland, CA)

We are growing our team at Bright Mill Care Services and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of about 487 people based in Oakland. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Support work on care plans with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with care plans. Working arrangement • Full-time, Monday to Friday. • On site in Oakland. • We will tell you what to expect at each step of the hiring process.
