Registered Nurse - care coordination (New Orleans, LA)

We are growing our team at Harborlight Ashton Care Services and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 95 people based in New Orleans. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on care plans with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in New Orleans, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
