Registered Nurse - care coordination (Milwaukee, WI)

We are growing our team at Greenridge Pike Early Learning and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with family communication. • BLS/ACLS certification. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Milwaukee. • We will tell you what to expect at each step of the hiring process.
