Registered Nurse - care coordination (Milwaukee, WI)

Summit Linden Care Services is looking for a Registered Nurse. The job centres on care coordination for caregiver training, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on caregiver training with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with caregiver training.
