Registered Nurse - care coordination (Milwaukee, WI)

We are growing our team at Clearpath Stone Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on fare systems with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Milwaukee. • We will tell you what to expect at each step of the hiring process.
