Registered Nurse - care coordination (Milwaukee, WI)

Clearpath Stone Transit Planning plans bus and rail service for cities around Milwaukee, and we need a Registered Nurse to lead our care coordination. Work is collaborative and map-heavy. What you will do • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on service alerts with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with service alerts. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Milwaukee, with some flexibility after the first few months. • We reply to every applicant.
