Registered Nurse - care coordination (Milwaukee, WI)

We are growing our team at Highbridge Haven Medical and adding a Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of more than 1,000 people based in Milwaukee. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on device records with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with device records. • BLS/ACLS certification. Working arrangement • Full-time, on a regular day shift. • On site in Milwaukee, with some flexibility after the first few months. • We reply to every applicant.
